The Transitional Assessment Template Exemplar - TATE - is a pioneering framework for transitional assessments that has been developed by Martin C Calder and built from the child’s world triangle (see child-focused and holistic). We have been concerned for some time that young people are often leaving placements with inadequate and often dated assessments that do not facilitate their transitional experience and this works against any progress they have made in addressing their abusive behaviours throughout their care episode.
The Transitional Assessment Template Exemplar consolidates previous assessment work and informs their current risk profile by embracing static, stable and dynamic risk factors and assets that can inform future wellness and care plans. It aims to provide the young person with a passport package as they emigrate from their time with us at Meadows and spread their wings for their onward journey.
